About the Author
Sidney Dekker (PhD Ohio State University, USA, 1996) is currently professor at Griffith University in Brisbane, Australia, where he runs the Safety Science Innovation Lab. He is also Professor (Hon.) of psychology at The University of Queensland, and Professor (Hon.) of human factors and patient safety at the Royal Children's Hospital in Brisbane. Previously, Sidney was Professor of human factors and system safety at Lund University in Sweden. After becoming full professor, he learned to fly the Boeing 737, working part-time as an airline pilot out of Copenhagen. Sidney is the best-selling author of a multitude of human factors and safety books, including, most recently, Safety Differently (2014), Second Victim (2013), Just Culture (2012), Drift into Failure (2011), and Patient Safety (2011).

It's definitely worth a read, but it seriously needs the assistance of a professional editor before the next revision. The message is in there, buried under layers of redundancy so thick you'd swear they had a target word count they were aiming for. I can sum it up thusly: "You can't understand why an accident occurred until you discover why the actions taken that led up to it made sense at the time. Pointing out mistakes made along the path to disaster is nowhere near as useful as figuring out why the people who made them didn't know they were mistakes".
This book is the perfect introduction to systems thinking when trying to understand accidents, improve safety, and make systems more resilient. The examples are great, and the author's perspective comes through loud and clear. He puts in clear relief the "old way" and "new way" of thinking about error, lays out his case for transitioning to the new way, and does it all clearly and concisely. Great read! I'll be buying extra copies to lend to colleagues.
I am an airline pilot and i have been greatly influenced by the work of Tony Kern. The Field Guide closed a gap on my knowledge about discipline and compliance. Five stars.
This book plus Dr. Conklin's pre accident investigations has completely altered my thinking. I've been in the safety field for 15+ years.. my thinking of human error was misaligned.. I strongly recommend this book to anyone in HSE or operations or businesses that hurt people
very helpful at understanding the language and intent of accident investigation and offers insight into the cause and effect of action taken in advance of adverse events
Quite an interesting take on accident investigation and understanding human error. The paradigm of the investigator placing themselves "inside the tunnel" to avoid hindsight bias is a powerful metaphor.Some of the chapters get to be a bit hard going - with lots of restating the basic tenet of Old View vs New View though.All in all - quite thought provoking and worth a read.
Found the book interesting and helpful. In am in Process Safety and it made me think about how I do HAZOPs and PHAs. This version is better organized than revision 2.
Looking forward to see the New View in progress. I would recommend this book to all involved in safety management and all the practitioners whom really cares about a safety culture and safe flights. We must understand that safety is in our hands and it's our responsibility to improve it as much as we can.
